MEDICAID SERVICE COORDINATION

 
General Information
Description of ProgramAdvocates for individuals and provides linkage to community supports and services that are consistent with the individual's needs and wants. Coordinators are well trained and versed with social services and medical and educational resources in Suffolk County. Also promotes the concepts of individual choice, individualized services and supports, and consumer satisfaction while using a person centered planning process in developing, implementing and maintaining an individualized service plan. The focus is on assisting each individual to achieve his or her unique desires and dreams in terms of having a circle of friends, a home, a job, enjoyable leisure and recreational activities, and access to medical and clinical services. Face to face meetings help facilitate reaching the individuals' desired goal.

Client Information
EligibilityPrimary diagnosis of developmental disability, Suffolk County resident, 20 years of age or older. Must be a medicaid recipient or call for fee information. Must meet OPWDD eligibility requirements.
