General Information
Description of ProgramOffers individuals with physical disabilities, 18 and over and seniors under the age of 65 residing in institutions or at home, home-care services aimed at fostering independent living in the most appropriate and least restrictive setting, usually home. Services include Service Coordination, Independent Living Skills Training, Home and Community Support Services, Structured Day Program(social model), Positive Behavioral Interventions, Environmental Modifications, Assistive Technology, Community Integration Counseling, Nutritional and Wellness Counseling, Education Services, Positive Behavioral Interventions and Support, Respiratory Therapy and Services.

Client Information
EligibilityIndividual ages 18-64 with a physical disability and must be medically frail and meet nursing home level of care, must have active Medicaid, choose to live in the community rather than a nursing facility, and must be able to be served with the funds and services available under the HCBS/TBI waiver and New York State Medicaid Plan. Referral Required.
