General Information
Description of ServiceAssists individuals and families to understand the importance of a health lifestyle and to prepare for the future. Individual programs are developed for women based upon their life course stage and level of need. Services include: the utilization of Community Health Workers, home-visiting, risk assessment, client centered service plans, referrals, health coordination, advocacy, preventive screenings and support. In addition a primary focus of the MICHC project is to enroll women of child-bearing age and their families in health insurance and assist these families to access appropriate medical care. Free Educational Workshops are offered continuously and reproductive life planning are education is provided to all participants.

EligibilityAll women of child bearing age (12-44) and their families who reside in Amityville, Bay Shore, Brentwood, Central Islip, Copiague and Wyandanch.
