General Information
Description of ProgramProvides a 24 hour community residence that serves men and women, 18-45 years of age, who suffer from mental illness. Services include daily living skills training, socialization skills, medication management, substance abuse prevention, assertiveness/self-advocacy training, case management, health services, and parenting education. Additional programs focus on community integration, vocational/educational skills development and rehabilitation counseling. Call for further information.

EligibilityNassau County residents 18-45 years old who are SSI/Medicaid eligible with primary diagnosis of psychiatric illness. May have secondary substance abuse diagnosis. (Mentally Ill Chemical Abusers-MICA) Referral required through SPA. Client must enter program voluntarily and must not be a danger to self or to others.
