General Information
Description of ProgramProvides low threshold case management for women who have experienced trauma, are at risk or diagnosed with HIV/Hepatitis B & C, or have a history of substance use. Facilitates access to medical/mental health care, trauma informed care, housing and benefits from DSS. Offers HIV testing, mental health/substance use assessments and on-site support groups and workshops including life skills education and Seeking Safety.

EligibilityWomen (Nassau County residents), ages 18 and older, who are at risk or diagnosed with HIV. Call or walk in for more information.
