General Information
Description of ProgramProvides individual and group services including mental health and case management to individuals with serious and persistent mental illness who have not had success with traditional mental health clinics. Multi-disciplinary team includes a psyhiatrist, LPN, RN, family specitalist, vocational specialist, substance abuse specialist and peer who provide in home services.

Client Information
Eligibility18 and older with severe and persistent mental illness. Psych evaluation and comprehensive assessment must be sent through SPOA (Suffolk County Division of Community Mental Hygiene Services operates a Single Point of Access (SPOA) to refer an individual.
