Description of ServiceProvides housing units for homeless adults with disabilities and a history of high Medicaid usage as well as low income individuals and families. Services include medication management, crisis intervention, case management, assistance with medical concerns, community integration, life skills training, employment assistance, parenting skills training, financial management, education assistance, and advocacy. Client Information
EligibilityMust have a history of high Medicaid usage, a psychiatric disability, be homeless, have a 30% AMI, and be age 18 or older OR head of household age 18 and over with a 50-90% AMI.
