General Information
Description of ServiceProvides assistance to those in recovery, facing homelessness in Suffolk County. Peer outreach counselors assist each person to help them reestablish hope, take charge of their situation and get back on track. Services are mobile and can be provided in any setting where a person is most comfortable. Individuals receive peer support and are assisted with linkage to resources such as housing, self-help, mental health services, substance abuse, medical services, financial assistance, legal services, employment and benefits as needed. Individuals can also be provided with food from the pantry as well as assistance with clothing and other basic needs.

EligibilityPeople 18 and over who live in Suffolk County and who are low income and dealing with mental illness and who are homeless or potentially homeless.
