Description of ServiceProvides at risk youth who are entering or returning from residential or detention settings or are at risk of Juvenile Justice involvement, with a link to community based agencies in or close proximity to their communities. Agencies provide access to individual/family counseling and case management services.

EligibilityYouth ages 10-21 residing in: Freeport, Hempstead, Long Beach, New Cassel, Westbury, Massapequa, Roosevelt, and surrounding communities.

Hours of Operation Monday-Friday 9am-4:45pm
