Peer outreach counselors assist each person to help them reestablish hope, take charge of their situation and get back on track. Services are mobile and can be provided in any setting where a person is most comfortable. Individuals receive peer support and are assisted with linkage to resources such as housing, self-help, mental health services, substance abuse, medical services, financial assistance, legal services, employment and benefits as needed.
