EMERGENCY/TRANSITIONAL HOUSINGProvides emergency housing through the Supervised Family Residence (SFR). The SFR houses up to nine families each, living under the supervision of a case manager. Families are able to give each other peer support and encouragement, and everyone benefits from the homelike atmosphere. CHI also provides emergency housing to single adults in a Supervised Singles Residence (SSR), licensed by New York State, using the congregate model and providing intensive case management, including supervision from a psychiatric nurse practitioner. In addition, CHI in partnership with Suffolk County DSS, operates a service center in Patchogue open to homeless single adults. Clients at the Resource Center receive help with applying for benefits, addressing mental and physical health issues, education and finding employment. WARMBED-NASSAU COUNTY'S EMERGENCY SHELTER RELIEF PROGRAMProvides immediate emergency shelter for homeless individuals and families in Nassau County during the winter months. Program typically operates from the day after Thanksgiving through March 31 of the following year. If winter conditions persist beyond March 31st, the program will continue until warmer weather arrives. During operational hours, employees from WARMBED and the Department of Social Services (DSS) will make appropriate referrals and provide shelter placement. Once placed, the staff at DSS and the Office of Housing & Community Development will contact those who are homeless in order to access resources for permanent housing.
