    Programs that provide permanent affordable housing and supportive services for families whose current housing situation is unstable with the objective of preventing family homelessness, preserving families at risk for separation and reunifying families that have been separated. The program generally provides intensive case management involving regular home visits, development of a family care plan, and coordination of services identified by the plan which may include health care, substance abuse treatment, counseling and other mental health services, parenting skills training, family literacy services, tutoring, vocational training, money management and other life skills training, recreational activities, child care and transportation. Some programs focus specifically on families involved in the child welfare system or other family subgroups.

    HELP SUFFOLKOffers a 76-unit transitional housing facility for homeless families. HELP Suffolk is the largest housing facility for the homeless on Long Island. The complex sits on seven acres of property and consists of four residential buildings, a community building, two playgrounds and a baseball field. HELP Suffolk has served over 4,000 families and places approximately 60 percent of families into permanent housing. Since its inception, approximately 500 individuals have secured employment in over 120 companies throughout Suffolk County. Available services include: Medical Services; Employment Services; Mental Health Services; Housing Placement Assistance; Early Childhood Education/Daycare; After School Programs; Homework Help/Tutoring; Client Case Management; Recreation; After-Care Services and On-Site Security.     HOMELESS INTERVENTION PROGRAMOffers people help to obtain long term housing and maintain that housing once assistance being provided to them ends. Services include legal services, financial assistance, eviction prevention, credit counseling, budgeting skills housing location, mediation, security and utility deposits, utility and rent arrears payment, and stabilization services. Financial assistance and support is provided to participants for an 18 month period and re-certification every three months is necessary.
