|AT-HOME MENTAL HEALTH SERVICES||Offers services of psychiatric nurses and social workers to provide individual care program addressing both physical and mental health needs. Provides thorough assessment/evaluation of patient and home environment, supportive counseling, and teaching of medication regime/living skills. |CHILDREN'S CARE COORDINATION||Offers case management for Nassau County families with children diagnosed with a serious emotional disorder (SED). Creates and individualsized care plan to deliver apporopriate services to support the child and his/her family at achiving personal goals while remaining at home and in the community. |HOME AND COMMUNITY BASED SERVICES||Home and community based services are available to individuals with a diagnosis of mental health disorder and/or substance use disorder to help facilitate more independence in the community. These services are individualized and can be in addition to the current treatment services you are already receiving.
For individuals 21 and older with:
-Diagnosis of mental health disorder and/or substance abuse.
-Have a Medicaid managed plan.

|HOME AND COMMUNITY BASED SERVICES (HCBS) WAIVER PROGRAM||Prevents long term psychiatric hospitalization and out of home placement for youth who are living in the community and have significant mental health needs that cannot be met by traditional community programs. Provides core services including Intensive Care Coordination-assessment, goal planning, linkage to community resources and coordination of service plan to help youth and family function in their natural environments; and Crisis Response for 24-hour call intervention. Referrals are made through SPOA (Single Point of Access). |HOME BASE 1 PROGRAM (CCSI)||Works with children who have been diagnosed with psychological/mental health issues and are at risk of long-term hospitalization or placement in a residential facility. Offers home and/and or school visits on a weekly basis. Services include, but are not limited to, anger management, school advocacy, life skills, coaching, communication skills, job assistance, self-esteem and self-awareness. |IN-HOME CARE MANAGEMENT SERVICES||Provides in-home care management Services including completion of Medicaid and Snap recertification; assistance with long-term plans, locating senior housing. Acts as an advocate for clients who have had home health aide hours reduced or failed to obtain increase in needed hours. |LINK PROGRAM/HOME BASED CRISIS INTERVENTION||LINK is an intensive, short-term crisis intervention program that provides assistance to families in crisis by helping to stabilize their family life and facilitating linkages to therapeutic resources in the community for ongoing support. The program uses a psycho educational approach that includes cognitive, behavioral, and client-centered approaches. Promotes and encourages improved family and individual functioning by teaching families necessary skills to remain together and prevent psychiatric hospitalization for children and adolescents in Suffolk County.
Referrals are accepted primarily through Suffolk's Children's Mobile Crisis Team and Stony Brook University Hospital CPEP (Comprehensive Psychiatric Emergency Program).
