Services include: the utilization of Community Health Workers, home-visiting, risk assessment, client centered service plans, referrals, health coordination, advocacy, preventive screenings and support. In addition a primary focus of the MICHC project is to enroll women of child-bearing age and their families in health insurance and assist these families to access appropriate medical care.
