|OPEN DOORS TRANSITION CENTER AND PEER PROGRAM||The Open Door Program assists people living in nursing homes and intermediate care facilities receive individualized home and community based services.
The Transition Center helps people living in these facilities receive home and community based services. This includes information about the services and supports available in their local community. There are transition specialists at Independent Living Centers in counties all over New York State. Suffolk Independent Living Organization houses the Open Doors Transition Center and Peer Program for all of Long Island.
Transition specialists will meet with you and/or your family members to identify what services you will need and help you in getting those services.
Open Door's Peer Program is a group of people who have lived in a nursing home and intermediate care facility and are now receiving services and support locally available to them. |SAN SIMEON BY THE SOUND CENTER FOR NURSING AND REHABILITATION||Operating as an outreach to the community this not-for-profit organization affiliated with Eastern Long Island Hospital offers long term care 24/7, short term rehabilitation care and Adult Day Care Health care by experienced professionals in a clean, well maintained environment. Case/care management is provided by its professional staff which includes nurses, therapists, social workers and dietitians.
